1/30/2023 EnchantaVaughn Approved Behavior: Client appeared to be excited for her therapy today. Client presents positive facial expressions and a good spirit. Client was seen interacting and engaging in appropriate conversation with her family but couldnât wait to show QMHP her new sneakers. Client verbalized she didnât have to do much to get gifts but is happy her mom spoils her. Client was ready to show off and ran to her room to change outfits. Client demonstrates excellent conduct in the home. Client verbalized she had an excellent day in school and received a compliment from her teacher. Client openly discussed her feelings about her current state and how she feels about going back to an in-person setting. Client admitted she enjoyed being at home but couldnât wait to make new friends at her new school. Client became cheerful when she realized she didnât have to return to her old classes and teachers. Client knew it was going to take a second to adjust to a different routine but have prepared herself for this transition. Client remained on task and in cooperation with home rules. 2. Behavior: Client appeared to be in a good mood as evidence by her participating in the family activity without conflict. Client conducted herself in a very respectable manner and followed all directions during the second part of session. Client was receptive to the idea of finding more activities that were fun for her but still incorporated therapeutic skill sets in the home. Client displays positive attitude and low symptoms of ADHD. Client is observed being polite and courteous. Client social behaviors were viewed as safe and desirable. Client looked very focused and attentive during the session, as evidenced by her productive and cooperative manners. Clientâs guardian communicated with counselor that she likes the idea of engaging in role play with client because she feels as if client would better understand things better and begin to implement them safely. Client presented a good attitude throughout the session. Client appeared to be in a good space and dealing with her personal challenges, as reflected in her growth and new development. 1. Intervention: QMHP reflected with client on her day, reviewing both good and bad experiences. QMHP focused session on explaining her treatment plan to client and mom. QMHP was able to get each family member to communicate with a calm voice tone, by issuing rules before talking. QMHP reminded the family that the rules consist of allowing each family member to speak without interruption. QMHP modeled proper positive behavioral expectations for the client to encourage additional layers of support and understanding of rules and expectations. QMHP explored clientâs responses and provided appropriate feedback. QMHP supported client making good decisions and having positive behaviors in the home. QMHP assisted client how to identify more effective ways to improve self-esteem. QMHP assisted the parent on how to specifically define and identify problem behaviors, identify her reactions to the behavior, determine whether the reaction encourages or discourages the behaviors, and generate alternatives to the problem behavior. QMHP assisted the parent on how to implement key parenting practices consistently, including establishing realistic age appropriate rules for acceptable and unacceptable behavior, prompting of positive reinforcement to encourage more positive behavior. 2. Intervention: QMHP reminded client of her strengths and abilities. QMHP praised client for her efforts and being respectful to her mom and sibling. QMHP will implement a routine that will encourage client to be responsible and promote self-confidence in therapeutic, academic, and social settings. QMHP modeled proper positive behavioral expectations for the client. QMHP supported client with displaying positive social interactions with peers and authority. QMHP introduced concept of nonverbal communication cues in efforts to review the expected behaviors or results to the nonverbal cue. QMHP continued to establish a positive supportive relationship with client. QMHP assisted client in monitoring her own behaviors in efforts to promote exercising self-control in focused areas such as respecting boundaries with others. QMHP modeled proper positive behavioral expectations for the client. QMHP reiterated with client the importance of maintaining a healthy relationship with her peers online and in person, without feeling she has to fit in with the everyone else. QMHP educated client about using her voice to uplift other young black girls. 1. Response: Client understands that her mother is trying to aid her in prioritizing skills and create structure in the home. Client mentioned that she has noticed mother has been making efforts on breaking the communication barrier in the home. Client also verbalized that her mother has also been consistent with her activeness in communicating with her and supporting her during this new transition. Client stated that mother has been praising and encouraging her when she displays positive and respectful behaviors. Client was able to reduce physical overactivity and become more attentive in the home. Client implemented appropriate social interaction skills. Client displayed an understanding of ADHD and its symptoms and causes. Client communicated positive affirmations and self-talk when talking about her day. Client was able to process making good choices with QMHP and her mother. Client exhibited techniques for appropriate self-expression and active listening. Client expressed a belief in self as being lovable and capable. Client demonstrated appropriate interpersonal skills, assertiveness, confidence in self and ability to problem-solve on her own. 2. During the session at the home, mother expressed clientâs behaviors have improved but she still has to remind client about remaining consistent with reacting positively when being reprimanded or redirected. Mother and client displayed good communication. Although mom is more stressed with the end of her pregnancy, she has cut back on work to spend more time with her daughter. Client responds well to praise and encouragemeResponse: The interventions were noted successful and making a difference, as the client displayed new habits and routines. Client utilized conflict management skills at home. Client was able to recognize personal strengths and abilities to perform better in the home. Client listed 2-3 behaviors that she was going to utilize in efforts to remain positive within her social environment. Client stated she loves playing outside after school and walking to the store to get snacks with her aunties. Client exhibited good manners after earning incentive for her hard work. Client focused on things she is good at and loves to do at home in order to stay motivated and make improvements. Client did a good job ignoring distractions and maintaining appropriate behavior in the home, reducing ADHD. Client was sociable and friendly in regard to the QMHP support. Client have adapted and is observed modeling appropriate behaviors in the home. Client stated she was glad to be in therapy both at home and in school because sheâs learning a lot about herself. Client expressed positive thoughts about using her voice to uplift young black girls her age.
